There are two primary ways that data has been partitioned inside legacy data management systems: Shared-data databases: an architecture that assumes all database cluster nodes share a single partition. Inter-node communications are used to synchronize update activities performed by different nodes on the cluster. Shared-data data management systems are limited to single-digit node clusters. Shared-nothing databases: an architecture in which all data is segregated to internally managed partitions with clear, well-defined data location boundaries. Shared-nothing databases require manual partition management. In virtual partitioning, logical data is abstracted from physical data by autonomously creating and managing large numbers of data partitions (100s to 1000s). Because they are autonomously maintained, the resources required to manage the partitions are minimal. This kind of massive partitioning results in: Partitions that are small, efficiently managed, and load-balanced. Systems that do not require re-partitioning events to define additional partitions, even when the hardware is changed. “Shared-data” and “shared-nothing” architectures allow scalability through multiple data partitions and cross-partition querying and transaction processing without full partition scanning. == Horizontal data partitioning == Partitioning database sources from consumers is a fundamental concept. With greater numbers of database sources, inserting a horizontal data virtualization layer between the sources and consumers helps address this complexity.     The graphics address remapping table (GART), also known as the graphics aperture remapping table, or graphics translation table (GTT), is an I/O memory management unit (IOMMU) used by Accelerated Graphics Port (AGP) and PCI Express (PCIe) graphics cards. The GART allows the graphics card direct memory access (DMA) to the host system memory, through which buffers of textures, polygon meshes and other data are loaded. AMD later reused the same mechanism for I/O virtualization with other peripherals including disk controllers and network adapters. A GART is used as a means of data exchange between the main memory and video memory through which buffers (i.e. paging/swapping) of textures, polygon meshes and other data are loaded, but can also be used to expand the amount of video memory available for systems with only integrated or shared graphics (i.e. no discrete or inbuilt graphics processor), such as Intel HD Graphics processors. However, this type of memory (expansion) remapping has a caveat that affects the entire system: specifically, any GART, pre-allocated memory becomes pooled and cannot be utilised for any other purposes but graphics memory and display rendering. Since PCI Express, the GART is extended to the GTT (Graphics Translation Table), which act as a buffer or cache between system memory and graphics card, and in PCI Express, the GTT buffer size is changeable by the GPU driver. == Operating system support == === Windows === Support for AGP GART was added since Windows 95 OSR2. The security of space can be affected by attacks such as disruption, corruption as well as the destruction of depended-upon assets/collected data. Government (e.g. militaries) and non-government sectors (e.g. financial industries) have started to become more reliant on numerous space-based services. Due to the criticality of these services, space security experts have identified these assets as high-value targets (HVT) that can cause detrimental consequences to all of Earth. == Scope and definitions == Space assets are broken down by three sub-sectors: the space component, the ground component, and the individual user component. The architecture of space assets is extremely complex and allows for a frequent attack vector utilized, the disruption by radio frequency (RF) cyber-attacks. In 2020, a memorandum was published by President Donald Trump, Space Policy Directive‑5 (SPD‑5). It established principles to ensure the safeguarding of all space assets. In 2023, the National Institute of Standards and Technology’s (NIST) published IR 8270, Introduction to Cybersecurity for Commercial Satellite Operations. This report established a baseline risk-management framework (RMF) to be implemented into space operations. == History == During the Cold War in the 1950s-1960s, the United States and Russia entered what was called the “Space Race”. By 1957, the Soviet Union successfully launched the first satellite into space named Sputnik. By 1961, the first key milestone was accomplished when the Soviet Union’s Yuri Gagarin became the first human to orbit Earth. This was later followed by the first American, Alan Shepard, to be launched into space; this was followed by John Glenn becoming the first American to orbit Earth in 1962. In 1969, a pinnacle milestone was reached when Apollo 11 launched into space and Neil Armstrong became the first man to walk on the moon. As space operations furthered, Commercial off-the-shelf products became increasingly popular but resulted in a rapid increase to the cyber-attack surface. Public awareness of space security did not increase until 2022, when the Viasat KA-SAT incident occurred, resulting in the disruption of a large number of modems across Europe. The attack was later accredited to Russia by the U.S. and the U.K. Policy and standards started to rapidly increase by 2020. The establishment of SPD-5 was released in 2020 followed by asset hardening instructions in 2022, and NIST’s IR 8270 in 2023. It was not until 2025 that Europe published their own findings in the Space Threat Landscape 2025 Report. This document led to the EU’s security proposals and standards. == Threats == === Radio-frequency Interference and Global Navigation Satellite Systems (GNSS) Spoofing === Space services are highly dependent on RF links for systems such as GNSS, however, a consequence of this dependency on RF is denial of service and deception. In 2017, the Black Sea maritime event occurred when numerous ships were subject to spoofing. Space services depend on RF links susceptible to jamming (denial) and spoofing (deception), including for GNSS/Positioning, Navigation, and Timing (PNT). Annotated incidents include the 2017 Black Sea maritime spoofing event affecting numerous ships, and extensive aviation GNSS spoofing patterns surveyed in various regions during 2024–2025. === Network intrusion and malware === Cyber threats can intrude and infect assets with malware. They do this by finding misconfiguration vulnerabilities, remote-management interfaces, and/or supply-chain vulnerabilities mainly in ground networks and user terminals. When KA-SAT occurred, it resulted from bulk modem disturbances. Forensic analysts later suggested malicious management controls and wiper malware as the root cause. === Supply-chain and lifecycle risks === The outsource of COTS components, external vendors, and software defined payloads allowed for vulnerabilities to emerge in the System/Product Lifecycle. In response, EU recommended the implementation of lifecycle-wide controls as mitigating factors. === Espionage, disruption, and influence === As Advanced Persistent Threats (APTs), Global Positioning System (GPS) intervention, and information warfare increased, assets like transponders became more frequent targets of attack. == Noteworthy incidents == The Viasat KA‑SAT incident of 2022, where a large number of modems in Europe were disrupted, resulted in the loss of telemetry access to a significant amount of wind turbines in Germany. The mass GNSS deception of the Black Sea in 2017 affected numerous ships when they started to convey fake central locations in Russia. Between 2024 and 2025, there was a mass, repetitive aviation GNSS spoofing that affected the aircraft of various regions. == Standards, guidelines, and best practices == SPD‑5 (U.S.) – This established risk-based engineering, verifying and ensuring positive control, and the implementation of risk mitigation controls. NIST IR 8270 – This created a RMF for COTS satellites. CISA/FBI SATCOM Advisory (AA22‑076) – Provided guidance on hardening techniques such as least-privileged, access control, encryption, etc.). ENISA Space Threat Landscape 2025 – It established the categorization of assets to organize threats, ensuring the observation of system/product lifecycle, and an RMF for COTS satellites. ECSS‑E‑ST‑80C (2024) – This established a standard for securing lifecycles in space, covering all segments (e.g. ground, launch, etc.). == Regulation and governance == As of 2025, there is no international regulations established for space assets, but the U.S., EU, and ESA institutional initiatives have published standards to address security concerns. The U.S. implemented SPD-5 and the Federal Communications Commission (FCC); the FCC addressed orbital debris. While the EU created standards to address technological mandates and support the implementation of NIS2. Lastly, the ESA created a special operations center to safeguard their satellites. International governance is still evolving, but forums have been held by the United Nations Committee on the Peaceful Uses of Outer Space. International conversations under forums such as the UN Committee on the Peaceful Uses of Outer Space (COPUOS) progressively note the cyber–space safety relationship, though formal global norms specific to space cybersecurity continue evolving. == Risk management approaches == Through RMF, mitigation controls have been implemented to reduce the risk of exploitation while increasing the security of space.     Hierarchical navigable small world (HNSW) is an algorithm for approximate nearest neighbor search. It is used to find items that are similar to a query item in a large collection, without comparing the query with every item one by one. The algorithm is commonly used for searching vector data. In these systems, an item such as a document, image, song, or user profile is represented by a list of numbers called a vector. Items with similar vectors are treated as similar according to the model that produced the vectors. HNSW provides a way to search these vectors quickly, especially in large datasets. HNSW stores vectors in a graph. Each vector is a node, and links connect it to some nearby vectors. The graph has several layers: upper layers contain fewer nodes and act like a rough map, while the bottom layer contains all nodes and gives a more detailed view. A search starts in an upper layer, follows links toward nodes that are closer to the query, and then repeats the process in lower layers until it finds a set of likely nearest neighbors. == Background == The nearest neighbor search problem asks which items in a dataset are closest to a query item. A direct search can compare the query with every item in the dataset, but this becomes slow when the dataset is large. Exact search methods based on spatial trees, such as the k-d tree and R-tree, can also become less effective for high-dimensional data, a problem often associated with the curse of dimensionality. Approximate nearest neighbor methods trade some exactness for speed or lower resource use. Instead of always guaranteeing the exact closest item, they try to return close items quickly. Other approximate methods include locality-sensitive hashing and product quantization. HNSW builds on research into small-world networks and navigable graphs. In a small-world graph, most nodes can be reached from other nodes through a short chain of links. In a navigable graph, a search procedure can use local information to move toward a target. Jon Kleinberg's work on navigation in small-world networks is an important example of this research area. Later work studied ways to add links that make graphs easier to navigate greedily. The HNSW algorithm extends earlier navigable small world methods for similarity search by adding a hierarchy of graph layers. This hierarchy helps the algorithm find a good region of the graph before doing a more detailed search in the bottom layer. == Algorithm == HNSW is based on a proximity graph. In this graph, nearby vectors are connected by edges. The algorithm uses these edges to move through the dataset, rather than scanning every vector. The graph is hierarchical. Every vector appears in the bottom layer. Some vectors are also placed in higher layers, with fewer vectors appearing as the layers go upward. The upper layers allow long-range movement across the dataset, while the lower layers allow a more detailed search near promising candidates. A typical search proceeds as follows: The search begins from an entry point in the highest layer. At each step, the algorithm looks at neighboring nodes and moves to a neighbor that is closer to the query. When it cannot find a closer neighbor in that layer, it moves down to the next layer. In the bottom layer, it explores a wider set of candidate nodes and returns the nearest candidates found. This search strategy is often described as greedy navigation. The algorithm repeatedly chooses locally better nodes, using the graph structure to approach the query point. == Construction and parameters == The HNSW graph is built incrementally. When a new vector is inserted, the algorithm assigns it a maximum layer, searches for nearby existing nodes, and connects the new node to selected neighbors in each layer where it appears. Implementations usually expose parameters that control the trade-off between speed, accuracy, memory use, and construction time. A higher number of graph connections can improve recall but requires more memory. A larger search candidate list can improve accuracy but makes queries slower. A larger construction candidate list can improve the quality of the graph but makes index building slower. Because HNSW is approximate, its results are not always identical to a full exact search. Its practical performance depends on the dataset, distance measure, implementation, and parameter settings. Benchmarking studies have found HNSW-based libraries to be strong performers among approximate nearest neighbor methods, although worst-case performance can differ from performance on common benchmark datasets. == Use in vector search systems == HNSW is used as an index in systems that store and search high-dimensional vectors. These systems include vector databases, search engines, and database extensions. Typical uses include semantic search, recommender systems, image similarity search, and retrieval-augmented generation. Several software projects implement or support HNSW. Libraries include hnswlib, which is associated with the original HNSW authors, and FAISS. There are few exceptions to this rule: most notably, Saturday! in Russia, which places their DOG in the top center. Many news broadcasters, as well as a few television networks, also place a clock alongside their bug. In the United States, Canada, Australia, and New Zealand, DOGs may also include the show's parental guideline rating. In Australia, this is known as a Program Return Graphic (PRG). It has become common to place text above the station's logo advertising other programs on the network. In many countries, some TV networks insert the word "live" near the DOG to advise viewers that the program is live, rather than pre-recorded. During televised sports events, a DOG may also display game-related statistics such as the current score. This has led people in Canada and the United States to refer to such a DOG as a score bug. Israeli channel watermarks most often appear on the top left or the top right corner since Israeli cable and satellite-based services often have the channel description and programming (OSD) on the bottom of the screen. Most channels have an opaque, full-color watermark, though exceptions exist, for example Channel 9, which displays a blue-tinted semi-transparent logo. In ad breaks, it is required to replace the channel watermark with another symbol – sometimes on the other edge of the screen – indicating there are ads at the moment. The Israel Broadcasting Authority, whose channels placed their logos in the top left corner, ceased broadcasting in May 2017. The new public broadcaster, the Israeli Public Broadcasting Corporation, displays its logos at the top right instead. Like TV channels from neighbouring Malaysia, all DOGs are removed during advertisement breaks. === Cambodia === Cambodian TV channels introduced DOGs in 1995. Like Thailand, all logos are full-color and displayed on the top-right corner of the screen. Some channels such as TV5 hide their logos during commercial breaks. China introduced logos in 1983 on the bottom-left corner of the screen, but they were used only during commercial breaks and clock idents. Later China Central Television (CCTV) introduced permanent DOGs for all programs in 1992, on the top-left corner of the screen. Hong Kong DOGs can be either of full color or semi-transparent and (except for RTHK 31) always be hidden during commercial breaks. Television Broadcasts Limited (TVB) placed their logos at the top-right corner of the screen while now-defunct Asia Television and other channels placed their logos at the top-left corner of the screen. Sometimes, weather information, date, and time clocks had been used alongside DOGs in news programs, continuity & live broadcasts. === India === The first on-screen logo in India was introduced in 1984 by DD2 Metro (now DD News). It was white and slightly transparent. All Indian TV channels have on-screen logos. They are always full-colors, never transparent, and they are almost never removed during commercial breaks (though the channels of the South Indian Sun TV Network did so until 2015). The great majority of Indian TV channels place their logos in the top right corner of the screen, though there are exceptions. The corner used may be broadcaster-dependent. Among the big national broadcasters: Channels from the Sony network always use the top right corner, without exception. Star channels also use the top right, with the exception of National Geographic and Nat Geo Wild, which use the top left corner in line with their international counterparts. Unlike most other major networks, the Zee Network's non-news channels containing 'Zee' in their name display their logos at the top left corner and not the top right. This has been the case since 15 October 2017, when almost all the Zee-branded TV channels of the Zee network rebranded with a new logo and, in many cases, a new graphics package and look. Before then, the logos were shown at the top right, as with other broadcasters. In 2018, Scroll signed its first partner websites, which included The Atlantic, Fusion Media Group, Business Insider, Slate, MSNBC, The Philadelphia Inquirer, and Talking Points Memo.     Big data primarily refers to data sets that are too large or complex to be dealt with by traditional data-processing software. Data with many entries (rows) offers greater statistical power, while data with higher complexity (more attributes or columns) may lead to a higher false discovery rate. Big data analysis challenges include capturing data, data storage, data analysis, search, sharing, transfer, visualization, querying, updating, information privacy, and data sources. Big data was originally associated with three key concepts: volume, variety, and velocity. The analysis of big data that have only volume, velocity, and variety can pose challenges in sampling. A fourth concept, veracity, which refers to the level of reliability of data, was thus added. Without sufficient investment in expertise to ensure big data veracity, the volume and variety of data can produce costs and risks that exceed an organization's capacity to create and capture value from big data. Current usage of the term big data tends to refer to the use of predictive analytics, user behavior analytics, or certain other advanced data analytics methods that extract value from big data, and seldom to a particular size of data set. "There is little doubt that the quantities of data now available are indeed large, but that's not the most relevant characteristic of this new data ecosystem." Analysis of data sets can find new correlations to "spot business trends, prevent diseases, combat crime and so on". Scientists, business executives, medical practitioners, advertising and governments alike regularly meet difficulties with large datasets in areas including Internet searches, fintech, healthcare analytics, geographic information systems, urban informatics, and business informatics. Scientists encounter limitations in e-Science work, including meteorology, genomics, connectomics, complex physics simulations, biology, and environmental research. The size and number of available data sets have grown rapidly as data is collected by devices such as mobile devices, cheap and numerous information-sensing Internet of things devices, aerial (remote sensing) equipment, software logs, cameras, microphones, radio-frequency identification (RFID) readers and wireless sensor networks. The world's technological per-capita capacity to store information has roughly doubled every 40 months since the 1980s; as of 2012, every day 2.5 exabytes (2.17×260 bytes) of data are generated. Based on an IDC report prediction, the global data volume was predicted to grow exponentially from 4.4 zettabytes to 44 zettabytes between 2013 and 2020. By 2025, IDC predicts there will be 163 zettabytes of data. According to IDC, global spending on big data and business analytics (BDA) solutions is estimated to reach $215.7 billion in 2021. Statista reported that the global big data market is forecasted to grow to $103 billion by 2027. In 2011 McKinsey & Company reported, if US healthcare were to use big data creatively and effectively to drive efficiency and quality, the sector could create more than $300 billion in value every year. In the developed economies of Europe, government administrators could save more than €100 billion ($149 billion) in operational efficiency improvements alone by using big data. And users of services enabled by personal-location data could capture $600 billion in consumer surplus. One question for large enterprises is determining who should own big-data initiatives that affect the entire organization. Relational database management systems and desktop statistical software packages used to visualize data often have difficulty processing and analyzing big data. The processing and analysis of big data may require "massively parallel software running on tens, hundreds, or even thousands of servers". What qualifies as "big data" varies depending on the capabilities of those analyzing it and their tools. Furthermore, expanding capabilities make big data a moving target. "For some organizations, facing hundreds of gigabytes of data for the first time may trigger a need to reconsider data management options. For others, it may take tens or hundreds of terabytes before data size becomes a significant consideration." == Definition == The term big data has been in use since the 1990s, with some giving credit to John Mashey for popularizing the term. Big data usually includes data sets with sizes beyond the ability of commonly used software tools to capture, curate, manage, and process data within a tolerable elapsed time. Big data philosophy encompasses unstructured, semi-structured and structured data; however, the main focus is on unstructured data. Big data "size" is a constantly moving target; as of 2012 ranging from a few dozen terabytes to many zettabytes of data. Big data requires a set of techniques and technologies with new forms of integration to reveal insights from datasets that are diverse, complex, and of a massive scale. Variability is often included as an additional quality of big data. A 2018 definition states "Big data is where parallel computing tools are needed to handle data", and notes, "This represents a distinct and clearly defined change in the computer science used, via parallel programming theories, and losses of some of the guarantees and capabilities made by Codd's relational model." In a comparative study of big datasets, Kitchin and McArdle found that none of the commonly considered characteristics of big data appear consistently across all of the analyzed cases. For this reason, other studies identified the redefinition of power dynamics in knowledge discovery as the defining trait. Instead of focusing on the intrinsic characteristics of big data, this alternative perspective pushes forward a relational understanding of the object claiming that what matters is the way in which data is collected, stored, made available and analyzed. === Big data vs. business intelligence === The growing maturity of the concept more starkly delineates the difference between "big data" and "business intelligence": Business intelligence uses applied mathematics tools and descriptive statistics with data with high information density to measure things, detect trends, etc. Big data uses mathematical analysis, optimization, inductive statistics, and concepts from nonlinear system identification to infer laws (regressions, nonlinear relationships, and causal effects) from large sets of data with low information density to reveal relationships and dependencies, or to perform predictions of outcomes and behaviors. == Characteristics == Big data can be described by the following characteristics: Volume The quantity of generated and stored data. The size of the data determines the value and potential insight, and whether it can be considered big data or not. The size of big data is usually larger than terabytes and petabytes. Variety The type and nature of the data. Earlier technologies like RDBMSs were capable to handle structured data efficiently and effectively. However, the change in type and nature from structured to semi-structured or unstructured challenged the existing tools and technologies. Big data technologies evolved with the prime intention to capture, store, and process the semi-structured and unstructured (variety) data generated with high speed (velocity), and huge in size (volume). Later, these tools and technologies were explored and used for handling structured data also but preferable for storage. Eventually, the processing of structured data was still kept as optional, either using big data or traditional RDBMSs. This helps in analyzing data towards effective usage of the hidden insights exposed from the data collected via social media, log files, sensors, etc. Big data draws from text, images, audio, video; plus it completes missing pieces through data fusion. Velocity The speed at which the data is generated and processed to meet the demands and challenges that lie in the path of growth and development. Big data is often available in real-time. Compared to small data, big data is produced more continually. Two kinds of velocity related to big data are the frequency of generation and the frequency of handling, recording, and publishing. Veracity The truthfulness or reliability of the data, which refers to the data quality and the data value. Big data must not only be large in size, but also must be reliable in order to achieve value in the analysis of it. The data quality of captured data can vary greatly, affecting an accurate analysis. Value The worth in information that can be achieved by the processing and analysis of large datasets. Value also can be measured by an assessment of the other qualities of big data.     A box blur (also known as a box linear filter) is a spatial domain linear filter in which each pixel in the resulting image has a value equal to the average value of its neighboring pixels in the input image. It is a form of low-pass ("blurring") filter. A 3 by 3 box blur ("radius 1") can be written as matrix 1 9 [ 1 1 1 1 1 1 1 1 1 ] . {\displaystyle {\frac {1}{9}}{\begin{bmatrix}1&1&1\\1&1&1\\1&1&1\end{bmatrix}}.} Due to its property of using equal weights, it can be implemented using a much simpler accumulation algorithm, which is significantly faster than using a sliding-window algorithm. Box blurs are frequently used to approximate a Gaussian blur. By the central limit theorem, repeated application of a box blur will approximate a Gaussian blur. In the frequency domain, a box blur has zeros and negative components. That is, a sine wave with a period equal to the size of the box will be blurred away entirely, and wavelengths shorter than the size of the box may be phase-reversed, as seen when two bokeh circles touch to form a bright spot where there would be a dark spot between two bright spots in the original image. == Extensions == Gwosdek, et al. has extended Box blur to take a fractional radius: the edges of the 1-D filter are expanded with a fraction. It makes slightly better gaussian approximation possible due to the elimination of integer-rounding error. The example does not handle the edges of the image, which would not fit inside the kernel, so that these areas remain unblurred. In practice, the issue is better handled by: Introducing an alpha channel to represent the absence of colors; Extending the boundary by filling in values, ranked by quality: Fill in a mirrored image at the border Fill in a constant color extending from the last pixel Pad in a fixed color A number of optimizations can be applied when implementing the box blur of a radius r and N pixels: The box blur is a separable filter, so that only two 1D passes of averaging 2 r + 1 pixels will be needed, one horizontal and one vertical, for each pixel. This lowers the complexity from O(Nr2) to O(Nr). In digital signal processing terminology, each pass is a moving-average filter. Accumulation. Instead of discarding the sum for each pixel, the algorithm re-uses the previous sum, and updates it by subtracting away the old pixel and adding the new pixel in the blurring range. A summed-area table can be used similarly. This lowers the complexity from O(Nr) to O(N). When being used in multiple passes to approximate a Gaussian blur, the cascaded integrator–comb filter construction allows for doing the equivalent operation in a single pass.

In response, the Ministry of Foreign Affairs developed the MoFA Mitra app to digitize complaint handling, improve communication between embassies and citizens, and make emergency response faster and more accessible. == Features == The app includes several services and features for Nepali citizens abroad, including complaint registration, rescue coordination, embassy communication, and digital consular support services. However, the development and application of medical AI must rely on a large amount of medical data for algorithm training, and the larger and more diverse the amount of data, the more accurate the results of its analysis and prediction will be. However, the application of big data technologies such as data collection, analysis and processing, cloud storage, and information sharing has increased the risk of data leakage. In the United States, the rate of such breaches has increased over time, with 176 million records breached by the end of 2017. By 2024, the U.S. Department of Health and Human Services reported 725 large healthcare data breaches affecting approximately 275 million individual records in a single year, marking a significant escalation in both the frequency and scale of incidents. == Black market for health data == In February 2015 an NPR report claimed that organized crime networks had ways of selling health data in the black market. In 2015 a Beazley employee estimated that medical records could sell on the black market for US$40-50. == How data is lost == Theft, data loss, hacking, and unauthorized account access are ways in which medical data breaches happen. Among reported breaches of medical information in the United States networked information systems accounted for the largest number of records breached. There are many data breaches happening in the US health care system, among business associates of the health care providers that continuously gain access to patients' data. == List of data breaches == In February 2024, a ransomware attack on Change Healthcare, a subsidiary of UnitedHealth Group, compromised the protected health information of approximately 100 million individuals, making it the largest healthcare data breach in United States history. The attack disrupted claims processing for healthcare providers nationwide for several weeks. In May 2024, MediSecure suffered a cyberattack involving ransomware in Australia. In May 2021, the Health Service Executive in the Republic of Ireland was the victim of a cyberattack involving ransomware, in the Health Service Executive cyberattack, with admission records and test results present in a sample of the data reviewed by the Financial Times. In October 2018, the Centers for Medicare and Medicaid Services in the US reported that around 75,000 individual records had been affected by a data breach that took place through the ACA Agent and Broker Portal. In 2018, Social Indicators Research published the scientific evidence of 173,398,820 (over 173 million) individuals affected in USA from October 2008 (when the data were collected) to September 2017 (when the statistical analysis took place). In response, those organizations agreed to pay the United States Department of Health and Human Services a US$4.8 million dollar fine. In 2009 1 million people using BlueCross BlueShield of Tennessee had their data stolen == Regulation == In the United States, the Health Insurance Portability and Accountability Act and Health Information Technology for Economic and Clinical Health Act require companies to report data breaches to affected individuals and the federal government. Under the HIPAA Breach Notification Rule, covered entities must notify affected individuals without unreasonable delay and no later than 60 days after discovering a breach of unsecured protected health information. Breaches affecting 500 or more individuals must also be reported to the HHS Secretary and to prominent media outlets serving the affected state or jurisdiction within the same timeframe; HHS publicly lists these larger breaches on its breach portal, commonly known as the "wall of shame." Breaches affecting fewer than 500 individuals are reported to HHS annually, no later than 60 days after the end of the calendar year in which they were discovered. Health Information Privacy Health Insurance Portability and Accountability Act of 1996 (HIPAA). - 45 CFR Parts 160 and 164, Standards for Privacy of Individually Identifiable Health Information and Security Standards for the Protection of Electronic Protected Health Information. HIPAA includes provisions designed to save health care businesses money by encouraging electronic transactions, as well as regulations to protect the security and confidentiality of patient information. The Privacy Rule became effective April 14, 2001, and most covered entities (health plans, health care clearinghouses, and health care providers that conduct certain financial and administrative transactions electronically) had until April 2003 to comply. This security provision became effective April 21, 2003. The Health Insurance Portability and Accountability Act (HIPAA) is the baseline set of federal regulations governing medical information. It does three things: i. i. i.Establish a structure for how personal health information is disclosed and establish the rights of individuals with respect to health information; ii.Specify security standards for the retention and transmission of electronic patient information; iii.Need a common format and data structure for the electronic exchange of health information. California-Specific Laws California’s medical privacy laws, primarily the Confidentiality of Medical Information Act (CMIA), the data breach sections of the Civil Code, and sections of the Health and Safety Code, provide HIPAA-like protections, although the terminology is different. HIPAA establishes a federal "minimum standard" that applies where there are gaps in California law, and HIPAA also specifies that stricter state laws will override or supersede HIPAA. California's health care privacy laws apply to providers who provide personal health records (PHR), while HIPAA only applies when the provider providing the PHR is a business associate of a covered entity. Federal law does not grant individuals the right to file a lawsuit in the event of a data breach (only the Attorney General can file a lawsuit), but California law does. This means that California law sets a higher standard for medical privacy, and that individuals in California enjoy stronger legal protections and more ways to hold entities that violate their medical privacy accountable. In the UK, the legal framework for how patient data is cared for and processed is the Data Protection Act 2018 (DPA), which incorporates the EU General Data Protection Regulation (GDPR) into law, and the common law duty of confidentiality (CLDC). The data protection legislation requires that the collection and processing of personal data be fair, lawful and transparent. This means that the collection and processing of data as defined by data protection legislation must always have a valid lawful basis and must also meet the requirements of the CLDC. In the China, Article 18 of the "National Health Care Big Data Standards, Security and Services Management Measures (for Trial Implementation)" (National Health Planning and Development (2018) No. 23) promulgated by the National Health Care Commission in 2018 states, "The responsible unit shall adopt measures such as data classification, important data backup, and encryption authentication to guarantee the security of health care big data." However, the scope and definition of important data are not covered. Although the "Information Security Technology-Healthcare Data Security Guide" (the "Guide") issued by the National Standardization Committee also proposes that important data should be evaluated and approved in accordance with the regulations, there is likewise no definition of the connotation and definition of important data.
